Patellar Mobilization

Patellar Mobilization is a hands-on physiotherapy technique used to improve the movement and mobility of the kneecap, also known as the patella. The patella plays an important role in normal knee movement and helps the quadriceps muscle work efficiently during activities such as walking, climbing stairs, squatting, and standing.

When the patella becomes stiff or its movement is restricted due to injury, surgery, prolonged immobilization, swelling, or surrounding soft-tissue tightness, normal knee movement can become uncomfortable or limited. Patellar mobilization uses gentle, controlled manual techniques to help restore appropriate patellar movement and support overall knee rehabilitation.

How Patellar Mobilization Works

The physiotherapist first assesses the movement of the patella in different directions, including upward, downward, medial, and lateral movements. This assessment helps identify areas where the kneecap may be restricted.

During treatment, the physiotherapist stabilizes the leg and applies gentle, controlled gliding movements to the patella. The direction and intensity of the mobilization are selected according to the patient's condition and specific movement restriction.

The technique is performed gradually and within an appropriate range. It is not intended to force the kneecap into painful positions. Instead, the goal is to encourage comfortable movement and improve the mobility of the tissues surrounding the patella.

Conditions That May Benefit

Depending on the physiotherapist's assessment, patellar mobilization may be incorporated into rehabilitation for:

  • Knee stiffness
  • Reduced knee range of motion
  • Patellofemoral pain
  • Stiffness following knee surgery
  • Rehabilitation following certain knee injuries
  • Knee stiffness after prolonged immobilization
  • Reduced patellar mobility
  • Certain kneecap tracking problems
  • Tight tissues surrounding the knee
  • Difficulty bending or straightening the knee

Benefits of Patellar Mobilization

When clinically appropriate, patellar mobilization may help:

  • Improve kneecap mobility
  • Reduce knee stiffness
  • Improve comfortable knee movement
  • Support restoration of knee range of motion
  • Improve movement of surrounding soft tissues
  • Assist rehabilitation following appropriate knee injuries or surgery
  • Prepare the knee for therapeutic exercises
  • Improve overall knee function

Improved patellar mobility can be particularly useful when stiffness is preventing the patient from comfortably performing knee-bending or straightening exercises.

What to Expect During Treatment

Before treatment, the physiotherapist assesses your knee movement, swelling, pain, patellar mobility, muscle flexibility, and functional limitations.

You will generally lie comfortably with the leg supported and relaxed. The physiotherapist gently holds the patella and moves it in specific directions using controlled manual pressure.

You may feel gentle pressure or movement around the kneecap. Treatment intensity is adjusted according to your condition and comfort. If the knee is particularly sensitive following an injury or surgical procedure, the technique may be modified accordingly.

Patellar Mobilization After Knee Surgery

Patellar mobilization can sometimes be included in rehabilitation following certain knee surgeries, particularly when stiffness around the kneecap develops. However, the timing and technique must be appropriate for the specific procedure and stage of healing.

The physiotherapist considers surgical precautions, tissue healing, swelling, pain, and range-of-motion goals before introducing mobilization techniques.

Patellar Mobilization as Part of Knee Rehabilitation

Patellar mobilization is usually combined with active rehabilitation exercises rather than used as a standalone treatment. Depending on the patient's condition, the rehabilitation program may include:

  • Knee range-of-motion exercises
  • Quadriceps strengthening
  • Hamstring stretching
  • Hip and glute strengthening
  • Patellar tracking exercises
  • Balance and stability exercises
  • Functional movement training
  • Gradual return-to-activity exercises

At our physiotherapy clinic, Patellar Mobilization is performed following an individual assessment and under professional supervision. The treatment is tailored to your specific knee condition and rehabilitation stage, with the goal of improving mobility, reducing stiffness, restoring comfortable knee movement, and supporting a safe return to daily activities and exercise.
